Your Visit
Appointments and Clinic Hours
Our scheduling staff is available Monday through Friday from 9:00am to 5:00pm. If you cannot attend your scheduled appointment, please notify us at least 24 hours in advance so we can arrange to see patients with urgent needs. If you are new to our practice, please come fifteen minutes early to complete your health history, insurance and billing information.
Examination
Your first examination will be comprehensive, plan on being here for at least one hour. We shall do our best to complete all necessary tests, explain results, confer with you and make recommendations at that time. Before you leave our office every effort will be made to explain the treatment alternatives available for your condition. Please feel free to ask questions at any time.

Emergencies
We understand that emergencies often occur after regular office hours. We provide 24-hour, seven days per week coverage. Emergency calls are handled through the Radiology department at St. Mark's Hospital at 801-268-7290, or the St. Mark's Hospital Emergency Department at 801-268-7129; ask for the Interventional Radiologist on call to be paged.
Prescriptions
If you need a medication refill, contact your pharmacist. A prescription normally authorizes the pharmacist to fill a limited number of automatic refills. Routine refills will not be authorized on weekdays after 4:00pm or on weekends. Refills for narcotic pain medications cannot be made over the phone.
Billing and Insurance
Our practice accepts most insurance and health plans. Please bring your insurance cards and picture identification with you to our office. The patient is responsible for his/her deductible and any co-payment amount (usually 20%) at the time of service. The portion of fees not covered by insurance will be your responsibility and must be paid in full prior to your procedure. Cash, personal check, credit card or local debit card may be used to make payment. It is our desire to help those who need our services. We are happy to assist you with financial arrangements and appreciate your sharing any suggestions you may have with us.
